A money order from any source, or a certified bank check are the only acceptable
methods of payment for UNICOR Optics contemporary eyewear. Personal checks and
cash will not be accepted by UNICOR Optics and will be returned to the member.
Members should retain the (yellow) fourth copy of the contemporary eyewear ordering
form and the money order receipt for their records. Mail the contemporary eyewear
ordering form, along with the money order made out to UNICOR Optics in the amount
of the member's cost share, directly to:

Unicor Optics will mail the eyewear directly to the ship to address as shown on the
ordering form in two to four weeks. The member may then make arrangements for the
eyewear to be fitted at a Coast Guard clinic, if so desired.

Although refunds are not allowed, contemporary eyewear may be returned to UNICOR
Optics, at no cost to the member, within sixty days of receipt to correct any and all
discrepancies, e.g., shipping damage, prescription errors, ordering form errors and errors
of fabrication. Members are encouraged to call a UNICOR Optics representative if they
have any questions, using the number on the ordering form. Members must include a
home (with area code) or work telephone number (with area code) on the ordering form
so that UNICOR Optics may contact the number to correct any ordering errors.
7.
ACTION. Area and district commanders, commanders of maintenance and logistics
commands, unit commanding officers, and Commander, Coast Guard Activities Europe shall
insure widest dissemination and compliance with the provisions of this instruction.
8.
FORMS/CATALOG AND TRIAL FRAMES SET AVAILABILITY. Initial issue of
UNICOR Optics prescription ordering forms (FPI 54), catalogs, and display samples of
contemporary eyewear frames will be provided by the Federal Prison Industries to all Coast
Guard clinics. Additional forms may be obtained from UNICOR, Federal Prison Industries,
Inc., Federal Correctional Institution, P.O. Box 757, Butner, NC 27509-0757, using form
number FPI 54.
