PRIVACY ACT STATEMENT

Principal Purpose:
To provide a means of selection of and
accepting U.S. Coast Guard Auxiliary
Radio Stations to operate under U.S.
Coast Guard Authorization.
3.
Routine Uses:
Retained by directors of Auxiliary and
cognizant USCG group commanders as a
record of which radio facilities have been accepted
by the director.
4.
Disclosure:
Voluntary, however, the detailed information
requested on this form enables the Coast Guard to
select qualified facilities to operate under
authorization. Failure by the member to provide all
or part of the information will prevent the
acceptance of the radio facility by the director.
