APPLICABILITY. Form CG-1258 is used by the public to apply for initial documentation,
exchange of a Certificate of Documentation, or return of a vessel to documentation. Form
CG-1261 evidences facts of build and establishes initial title to a vessel. Forms CG-1340
and CG-1356 serve as evidence of transfer of title.

ACTION. Area and district commanders, and merchant vessel documentation officers shall
insure widest dissemination and compliance with the provisions of this instruction.
6.
FORMS AVAILABILITY. Forms CG-1258, CG-1261- CG-1340, and CG-1356 may be
procured from Supply Center, Brooklyn, using stock numbers as follows:
