PURPOSE. The purpose of this instruction is to announce revisions of the following forms:
Application for Initial Issue, Exchange, or Replacement of Certificate of Documentation;
Redocumentation, CG-1258; Builder's Certification, CG-1261; Bill of Sale, CG-1340; and
Bill of Sale by Government Entity Pursuant to Court Order or Administrative Decree of
Forfeiture; CG-1356.

DIRECTIVES AFFECTED. Previous editions of forms CG-1258, 1261, 1340, and 1356
are obsolete. Existing supplies of forms shall be disposed of in accordance with unit
recycling processes. Commandant (G-MVI-5) will make an initial distribution of forms to
documentation services and maritime law firms with a known interest in vessel
documentation. Documentation officers may continue to process obsolete forms previously
distributed to vessel owners and agents provided those forms contain all data necessary to
process the documentation case.

DISCUSSION. The revised documentation forms were devised in accordance with
references (a) and (b) in order to simplify the vessel documentation process.
